Getting to know you

First meeting questionnaire  •  1 of 3

Dear client,

Thank you for taking a few minutes to tell us what matters to you. Your answers help us prepare, so we can make the most of our first meeting. Fill in what you can. If there is anything you would rather talk about in person, please leave it blank. If you are coming as a couple, you can fill in one form together.

You and the meeting

1. What is your name, and what would you like us to call you?

2. Will someone else be joining you?

3. How can we contact you?

4. What are your ages?

What matters to you

5. What prompted you to contact us, and what would you most like help with?

If the options above don’t cover what you want, tell us what else you would like help with.

6. Is there an upcoming change or deadline we should know about?

Tell us what is happening and roughly when, or leave blank if there is nothing.

Your financial picture

First meeting questionnaire  •  2 of 3

This page is optional. Estimates or ranges are welcome, and unknown amounts can stay blank. Use Australian dollars. Enter household amounts once; give individual super balances where shown.

7. What best describes your work situation?

8. Does anyone rely on you financially?

9. Roughly how much comes in and goes out each month?

10. Your financial position at a glance

10.1 Do you have your own property?

This means the home you live in. Value: roughly what it would sell for today. Debt: what you still owe on the loan, without subtracting any offset account. Offset: the balance of any offset account linked to the loan.

10.2 Do you have any investment properties?

Properties you own but don’t live in, such as rentals or holiday homes, held outside super. If you have more than one, add them together. Value, debt and offset work the same way as above.

10.3 Do you have super? (you and your partner)

Your current balances from your latest super statements, including any SMSF.

10.4 Do you have any properties inside super?

For example, property owned by your SMSF. Its value is already included in your super balance above, so this just shows how much of it is property. Value, debt and offset work the same way as in 10.1.

10.5 What’s your bank balance?

The combined balance of your household’s everyday, savings and term deposit accounts. Leave out offset accounts, which go with the loans above.

10.6 Other investments you want us to know about

Investments held outside super, such as shares, managed funds or cryptocurrency. A rough value for each is enough.
